Use this tool to look up your invoicing deadline for the RHC programs.
Use this tool to learn if USAC has received your FCC Form 466 and collected the information it needs to make a funding commitment decision. If we are missing information, you will receive an Information Request once your form has been assigned to a reviewer.
The tool's dataset refreshes daily. If your form status does not appear in the report more than 5 days after submitting your FCC Form 466 in MyPortal, please contact the RHC Help Desk via email or at (800) 453-1546.
Use this tool to view information about funding decisions, total committed amounts, and total disbursement amounts for the Rural Health Care (RHC) Program. The data is collected from the FCC Form 462 (Funding Request Form, Health Care Connect Fund (HCF)), FCC Form 466 (Funding Request and Certification Form, Telecommunications program) and FCC Form 466A (Internet Service Funding Request and Certification Form, Telecommunications program). All information within the FCC Forms 462, 466, and 466A data collection is self-reported by applicants.
Use this tool to view service request information provided by individual Health Care Providers (HCPs) and consortia applying for funding through the Rural Health Care (RHC) Program. The data are collected from the FCC Forms 461 and 465, which includes the service request information. All data contained within the FCC Form 461 (Request for Services Form, Health Care Connect Fund (HCF) program) and 465 (Description of Services Requested & Certification Form, Telecommunications program) is self-reported by applicants to RHC.
Use this tool to identify the urban and rural rate for your site and service when completing your FCC Form 466 (Funding Request Form).
What rates will you use?
- Rural rates: HCPs must use the lower of the rural rate available in USAC’s database or the rate included in the service agreement that the HCP enters into with the service provider when requesting funding.
- Urban rates: HCPs must use the urban rate available in USAC’s database.
